FULL TIME BENEFITS

PART-TIME: Any employee who is scheduled to work a maximum of less than thirty (30) hours per week or on an “as needed” basis, and is not classified as a “temporary worker”. Part-time employees do no receive benefits.  
      
FULL-TIME: Any employee who is regularly scheduled to work a minimum of thirty (30) hours or more per week, and is not classified as a “temporary worker”.

PROBATIONARY PERIOD
The initial introductory period of employment is for a period of ninety (90) days. During this period and at any time thereafter, the company reserves the right to terminate the employment relationship for any reason or no reason. Any benefits to which the employee may be entitled will not begin prior to the successful completion of this period.

PERFORMANCE EVALUATIONS
A formal, written evaluation will be conducted annually on or near the anniversary date of your employment.
If business conditions and individual performance merit a pay increase, such an increase will be given following your evaluation. As a general rule, raises are not given outside of the annual evaluation.

In order to be eligible to receive vacation time, you must work for the company on a full-time basis for at least one year. Vacation time not used on an annual basis is forfeited – it does not carry over to the next year. Vacation reimbursement or a combination of work/vacation reimbursement may not exceed 80 hours in any two-week pay period. Upon termination of your employment relationship with Native Angels for any reason, any unused vacation is forfeited. This means that no reimbursement for unused vacation time will be given to you upon your departure from the company.

SICK TIME
Sick time is available to full-time workers after the end of the probationary period.
Upon termination of your employment relationship with Native Angels for any reason, any unused sick time is forfeited. This means that no reimbursement for unused sick time will be given to you upon your departure from the company.

INSURANCE     (Open enrollment month of of April)
All full time employees in good standing with this company will be offered Major Medical, Dental, Vision, Life Insurance and Short Term Disability after their initial 90 days probationary period. Eligible employee should complete application 2 weeks before probationary period ends. HR staff will provide specific information.

401-K RETIREMENT       (30 Hours per Native Angels Policy)
Open enrollment for retirement will be Jan. 1 and July 1 of each year. All eligible employees will be notified in advance. This is for full time employees only with 1 year service and in good standing with this company. Plan administration documents are available by appointment with the office manager at the main office.

BEREAVEMENT
2 days/calendar year for direct family including: husband/wife/children/mother/father/sister/brother/
maternal & paternal grandparents.

JURY DUTY/NATIONAL GUARD SERVICE
NATIVE ANGELS HOMECARE AGENCY cannot reimburse employees for absences due to jury duty or service in the National Guard. However, days served for either reason will not affect employment status.

RESIGNATION
Upon resignation or termination of employment, any missing documentation will be required to be submitted prior to the employee’s leaving, along with any company property held by you must be returned to Native Angels within twenty-four hours.

$50 / $ 25 CNA INCENTIVE PROGRAM FOR PERFECT ATTENDANCE

QUARTERLY BONUS CARD

Requirements: PRN not eligible
Full time $50
Part time $25

Must work 90 days scheduled workdays without being absent to qualify for the bonus.

PTO is available to full-time employee after the end oft the probationary period. (One day per Month) Written approval must be obtained from your supervisor prior to the onset of the PTO time. Upon termination of your employment relationship with Native Angels for any reason, any unused PTO time is forfeited. This means that no reimbursement for unused PTO time will be given to you upon your departure from the company.
