Native Angels offers a full range of home health care. We offer
services based on the individualized needs of the patient, in accordance
with the physicians orders.
Skilled Nursing
We offer 24 hour seven day per week
coverage for our patients. Our nurses develop individualized care
plans based on the physician’s orders and your needs as a patient.
We work extensively in providing education to both the patient and
the caregiver in the management of any medical condition. Our attention
to detail ensures that our patients receive the very best in home
health care. We also offer aide services to assist our patients with
bathing, feeding, meal preparations, etc.
Therapy: Physical, Occupational
Whatever the home therapy need, Native
Angels highly trained and compassionate
staff can assist you. Our therapists
are caring clinicians who tailor each program to fit the patient’s
needs. We focus on assisting our patient’s in regaining as much functional
independence as possible and we teach the family/caregiver to continue
once home health therapy ends.
HOW DOES HOME HEALTH CARE HAPPEN?

Diagnosis
Following an office
visit or hospital
stay, your physician
decides that home
health care is
required.

STEP 2 |
Discussing Options
Together with your physician and family, you decide on the
proper services and the right home health care agency to accommodate
your special needs. |

STEP 3 |
Developing a Plan
The home health care agency comes to your home, evaluates
your needs and develops an overall care strategy, which is submitted
to your doctor for final approval. This plan might also include
suggestions regarding social service, dietary needs (i.e. Meals
on Wheels), etc.

STEP 4 |
Your Home Care Visits
Each care program is strictly supervised by a registered
nurse or physical therapist to assure your health needs are being
met on a continuous basis. Your physician and home health care
agency work together to monitor progress and determine if and
when changes to the care plan are necessary.

When considering home health care providers, ask if the agency:
Is Medicare-certified. That means it is approved to provide services to patients with Medicare.
- Offers the specific health care services you need (i.e., physical therapy, occupational therapy, speech therapy, medical social worker, home health aides).
- Meets your special needs (like language preference), if necessary.
- Offers the personal care services you need (like help bathing, dressing, and
using the bathroom).
- Offers the support services you need (like help with laundry, cooking, shopping or housekeeping).
- Has staff available to provide the type and hours of care your doctor ordered, and can start when you need them.
- Is recommended by your doctor, or social worker.
- Has staff available at night and on weekends for emergencies.
Explained what your insurance will cover, and what you must pay out-of-pocket.
- Has letters from satisfied patients, family members, and doctors that testify to the home health staff providing good care.
