Our Mental Health Program, established in 2003, currently provides Community Based Services, Community Based Rehabilitative Services, Specialized Therapies, CAP-MR/DD Program, and an array of other services as well. Our Mental Health Staff have more than 10 years of experience with providing these services.
Defining Our Services:
Developmental Therapy: a service that includes individually designed instructions, training or intervention activities based on assessment of strengths/needs of the individual adult or child. This service is for consumers with a developmental disability diagnosis. This service is primarily for non-Medicaid consumers. Funding comes from state dollars. However, once funding is gone, no service can be provided until next fiscal year and/or the LME requests and receives additional money.
Community Support (child, adult and team): consists of mental health and substance abuse rehabilitation services and supports to assist persons achieving and maintaining rehabilitative sobriety and recovery goals. This service provides one-on-one interventions with the community to develop interpersonal and community coping skills, including adaptation to home, school and work environments.
Targeted Case Management: A service that assists individuals in gaining access to needed state plan services as well as needed medical, social, educational and other services. Case managers are responsible for coordinating, arranging, linking and monitoring of the provision of services in the Person Centered Plan. Case managers initiate and oversee the process of assessment and reassessment of the individual’s level of care.
Diagnostic Assessment: An intensive clinical and functional face-to-face evaluation of a recipient’s mental health, developmental disability or substance abuse condition which results in a recommendation regarding whether the recipient meets target population criteria, and includes an order for enhanced benefits services that provide the basis for the development of an initial person centered plan. The assessment is signed and dated by the MD, DO, PA, NP and licensed psychologist.
Occupational, Physical, and Speech Therapy: Primarily for birth to three year-old children
CAP-MR/DD (Community Alternative Program) A funding source provided as an alternative to being placed into an institution. Will provide services at home such as PCS, Respite, Supplies, etc.
Hourly Respite: Provides temporary relief to the caregiver.
Crisis Stabilization Program: Provides immediate evaluation, triage and access to acute mental health services; provides developmental disabilities and/or substance abuse services, while providing treatment and supports to effect symptom reduction; harm reduction; the program also safely transitions persons in acute crises to appropriate crisis stabilization and detoxification supports/services.
Hearing Impaired (interpreters available): Services provided for any consumer/guardian who may need interpretation.
Spanish Interpreters Available: Services provided for any Spanish speaking consumers/ guardians who may need interpretation.
Injections for Medications: Provided for psychotropic medications and administered by a Native Angels nurse. Private pay is an option.
Psychiatric Evaluations: Services provided by a clinical psychiatrist, to include medication monitoring, ordering of services, etc.
