Skilled Nursing
Our nurses are skilled professionals available to you 24/7 while providing IV therapy, palliative care, wound, cardiac and post surgical care.To ensure optimal health and independence we place much emphasis on teaching patients and their families how to continue with care once services have expired. Our attention to detail ensures that our patients receive the very best in home health care.
Therapy: Physical, Occupational and Speech
Our therapists are caring, highly trained clinicians who tailor each program to fit the patient’s needs. We focus on assisting patients in regaining as much functional independence as possible while teaching the family caregiver how to continue with care when therapy ends.

HOW DOES HOME HEALTH CARE HAPPEN?

Diagnosis Following an office visit or hospital stay, your physician decides that home health care is required. |
| STEP 2 |
Discussing Options Together with your physician and family, you decide on the proper services and the right home health care agency to accommodate your special needs. |
| STEP 3 |
Developing a Plan The home health care agency comes to your home, evaluates your needs and develops an overall care strategy, which is submitted to your doctor for final approval. This plan might also include suggestions regarding social service, dietary needs (i.e. Meals on Wheels), etc. |
| STEP 4 |
Your Home Care Visits Each care program is strictly supervised by a registered nurse or physical therapist to assure your health needs are being met on a continuous basis. Your physician and home health care agency work together to monitor progress and determine if and when changes to the care plan are necessary. |
